“The Caribbean Fantasy had been detained 3 times in the years immediately before the fire, yet no red flags went up,” commented NTSB Chairman Robert Sumwalt.

The National Transportation Safety Board has determined that the fire aboard the roll-on/roll-off car and passenger ferry Caribbean Fantasy was caused by leaking fuel that contacted a hot surface in the ship’s engine room.

The fire occurred on August 17, 2016, as the ferry was underway off San Juan, Puerto Rico with more than 500 people on board. The Master gave the order to abandon ship, prompting a major search and rescue operation.

The probable cause was determined during an NTSB meeting on the incident on Tuesday.

The investigation revealed that the second mate, who was alone on the bridge at the time of the accident, had fallen asleep after working a 6-on, 6-off shift schedule for at least two days leading up to the grounding.

The Transportation Safety Board of Canada (TSB) has released its investigation report into the October 2016 grounding and sinking of the American tug Nathan E. Stewart near Bella Bella in British Columbia, Canada, listing crew fatigue as the main factor in the incident.

The Board also determined that the spill response was adequate, but noted Canada’s spill response regime could be improved.

On 13 October 2016, shortly after 1 a.m. Pacific Daylight Time, the U.S-flagged articulated tug-barge (ATB) composed of the tug Nathan E. Stewart and a tank barge, DBL 55, went aground on Edge Reef near Athlone Island, at the entrance to Seaforth Channel, approximately 10 nautical miles west of Bella Bella, British Columbia, within the Heiltsuk First Nation territory.

In August 2017, the Chinese authorities placed 24 types of solid waste on an import ban list.

However, local correspondents have advised that in April 2018 the authorities announced the following changes:

16 types of solid waste including scrap metal, scrap ship and industrial waste will be added to the list as of 31 December 2018.

16 types of solid waste including stainless steel waste/scrap and wood waste/scrap will be added to the list as of 31 December 2019.

Importing such wastes without an appropriate licence will result in customs ordering the return of the waste and a fine of between RMB 100,000 and RMB 1 million (approximately US$ 15,000 and USS$157,000)
